USING MINI-IMPLANTS FOR TREATING MANDIBLE FRACTURES IN PATIENTS WITH AEDENTIA

Annotation:

The article consecrated one of the urgent problems Maxillofacial Surgery – Treatment of mandible fractures. Presents a new way of fixing bone fragments in uncomplicated mandibular fractures in older persons with full secondary aedentia. The necessity of using this technology in the rehabilitation of patients with mandibular fractures. The results of treatment of 219 persons with fractures of the mandible in the presence of full secondary aeden-tia, 96 of them were evaluated retrospectively. Rehabilitation of patients had used two different methods of treat-ment. This was the criterion for allocation of patients into groups: group 1 (73 persons), which is commonly used for the treatment of orthopedic treatment (splinting using bus port and parietal- submental bandage), group 2 (146 persons) – a method of using mini- implants. The study included elderly and very elderly (60 years and older) ac-cording to the International Classification of Diseases 10th Convocation 43 Assembly of WHO. In our research, there are cases of following locations: the area of the canines (32. 4%), the area of mental holes (28. 9%). In 38. 7% of the lower jaw was injured in the «custom» field. In the research was used mini-implant company Konmet (Russia) for fixing various musculo-containing ele-ments (alloys, rubber rods in the mouth, designed to immobilize fragments of jaws. Machine has been designed for the compression- distraction osteogenesis, moving teeth) – titanium screws, latches. The screws are made of pure titanium (U. S. Standard ASTM F 67-00), along with a unique biocompatibility characterized by sufficient mechani-cal strength and meet the quality standards BS EN ISO 9001:2000 and ISO 13485:2003 by the British Standards Institute (BSI), the UK, certified in Ukraine . Mini implants are easy to install, able to withstand the applied forces directly and minimally traumatic surrounding soft tissues of the mouth. Indications for use by single or double mandibular fractures without displacement of fragments or easily doable reposition and fixation of support when performing osteosynthesis. Contraindications to install mini – implant were: the presence of granulomas, cysts or tumors in the area of the intended installation of mini- implants. Using of mini-implants in combination with Port’s tire for treatment of mandible fractures in older persons with full secondary aedentia helps restore bone function and masticatory system in up to three months. Positive dynam-ics is observed from the first week after fixing the design, while using traditional treatment promotes bone formation and reduce the diameter of the fracture gap to 21 days. Immobilization of the mandible with help of the proposed technique reliably fixing bone fragments and optimize the posttraumatic course of the process.
