Odontogenic phlegmons of maxillofacial area are one of principal problem of surgical dentistry and maxillofacial surgery. Draining of purulent wounds is one of the most popular methods of treatment of phlegmons and abscesses which is taken to providing of outflow from a wound. Objective: to conduct the comparative estimation of efficiency of draining of odontogenic phlegmons of maxillofacial area by traditional methods and by the use of device for draining of purulent wound of own construction, utilizing a ball scale. Materials and methods. Conducted general clinical and laboratory research methods of 64 patients with odontogenic phlegmons of maxillofacial area: 43 ( 67,2% ) men and a 21 ( 32,8% ) woman in age from 18 to 55 years. Patients were divided into two groups: the first main group (31 patients) included patients with odontogenic phlegmons who was treated by traditional methods of draining, the second (comparison group – 33 patients), patients with odontogenic phlegmons who was treated by the use of device for draining of purulent wound of own construction. All patients were without concomitant pathology. For the estimation of intensity of inflammation at the patients were chosen and appraised next signs: presence and intensity of pain in the area of inflammation, degree of expressed of inflammatory contracture to the lower jaw, presence and intensity of infiltration, presence, volume and character of exudates in the wound after excision, character of granulations. Results. Grade-rating system for evaluation of the pyoinflammatory diseases severity of the maxillofacial area in different types of drainage. To assess the severity of the inflammatory processes we suggested the use of local symptom complex: pain in the inflammation region, inflammatory contracture of the lower jaw, severity of soft tissues edema, wound exudate volume. Peripheral blood criteria: leukocytosis, erythrocyte sedimentation rate (ESR). The degree of reparative regeneration processes in the postoperative wound, more specifically the nature and amount of granulation also were evaluated. Sixty-four patients were included in current study. A subgroup of 31 patients, which were treated with device for purulent wounds drainage of our recommendation were formed. Remaining group of 33 people were treated with traditional pipe drainage. Our data allow predicting and appraising the flow pattern of purulent process. The method of draining offered by us for the complex treatment of odontogenic phlegmons is more high-quality and more effective, as compared to the traditional methods of draining, that is confirmed results of research. The use of the modified grade-rating scale of estimation of severity of pyoinflammatory processes of maxillofacial area in a dynamics is significant and allows to determine that at extension of purulent process (index of diagnostic test more than 1,0) the correction of complex treatment both medicinal and surgical is needed. The conducted researches determine perspective direction of development of new methods of draining of purulent wounds; improve efficiency of treatment at patients with pyoinflammatory processes in a maxillofacial area.
