The Study of Clinical and Economical Efficiency of the Improved Method of Immediate Prosthetics

Introduction. To eliminate the complications of the partial edentulism and to improve the conditions for rehabilitation of patients is possible by the immediate removable prosthetics. The immediate dentures made by non-flask method proposed by Golubnichiy A. P. (1979) with the use of self-cured base acrylates does not always correspond to functional and aesthetic requirements. We at the Department of Prosthetic Dentistry of KhNMU im- proved the method of the immediate and transitional prosthetics with the use of self-cured base acrylate «Redont- colir» with an extended range of colors that enables the rehabilitation of patients in a single clinical visit without a dental technician with the individualization of the basis’s color and setting of the artificial teeth according to the natural teeth before extraction. The aim of this study is to determine the clinical and economical efficiency of the improved method of immediate pros- thetics with the use of self-cured base acrylate “Redont-colir” with an extended range of colors. Materials and methods. Altogether for the 25 patients of the main and control group were made 39 immediate remov- able dentures, 20 dentures were manufactured by improved method in a single clinical visit and 19 by Golubnichiy A. P. in two clinical visits. For evaluation of the clinical and economic efficiency (CEE) of each of the two methods was calculated the integral index by modified method of Yanishen I. V (2004) by the formula: CEE=CFE+WT+EE where CEE is integral index of the clinical and economical efficiency of methods of dentures` manufacturing; CFE – clinical and functional efficiency of the prosthetics; WT – working time spent by dentist and dental technician in making one denture; EE – economical efficiency of the immediate prosthetics` method. Results of the study and their discussion. By the index of clinical and functional efficiency the improved method of the immediate prosthetics exceeded the method by Golubnichiy A. P. in 2,7 times; by the time efficiency – in 3,3 times due to the absence of dental technician`s working time, by the index of economical efficiency the improved method of the immediate prosthetics is cheaper than the method by Golubnichiy A. P. in 1,7 times. In general by the index of clinical and economical efficiency the improved method of the immediate prosthetics exceeded the method by Golubnichiy A. P. in 7,7 times. The immediate denture made by improved method does not require a significant correction of the articulatory- occlusal relationships, the inner surface of its basis is fully consistent with the prosthetic bed`s relief at the time of imposition of the denture, protects the postoperative wound from the external influences, and at the same time forms the prosthetic bed for future prosthetics, keeps the height of bite, prevents the secondary deformation, restores norms of aesthetics, speech, thereby rehabilitate the patient socially. Conclusions. By comparison of the results of the clinical and economical effectiveness of the improved method and method by Golubnichiy A. P. it was found that the first one dominates in 7,7 times. A perspective for further research. These results argue the feasibility of further development of constructional acrylic materials and of improving the methods of their use for transitional and immediate prosthetics.
