LEVEL OF ORGAN-SPECIFIC ENZYMES IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE IN COMBINATION WITH HYPERTENSION

In order to study the activity of marker organ-specific enzymes and the dynamics of their changes were examined 145 patients which were divided into 3 groups: the main group included patients with COPD in combination with hypertension (55 persons); comparison group consisted of patients with isolated COPD (45 people); group control made relatively healthy patients (45 people). Was studied the activity of creatine phosphokinase and creatine kinase-MB fraction, gamma-glutamyl transpeptidase, lactate dehydrogenase, aspartate aminotransferase and alanine aminotransferase, α-amylase and alkaline phosphatase by biochemical research method. Statistical analysis of the results of research carried out with use of computer applications for packet processing statistics Statistica 6.1 (StatSoft, Inc., USA). For the critical level of significance when testing statistical hypotheses accepted p<0.05. Analysis of the survey results organ-specific activity of enzymes in the serum of patients with COPD combined with hypertension showed increased activity CPK increased by 146.07%, CK-MB at 190,05%, γ-GT at 207.43%, LDH 139, 14% to 381.54% of AST, ALT at 270,74%, α-amylase to 102.52% and alkaline phosphatase at 85.57% compared to the control group. Receiving data indicating the development of membrane pathology, accompanied by the arrival of tissues and organs in the bloodstream structured membrane enzymes of energy, protein, carbohydrate metabolism. The study of enzyme activity in patients with COPD showed CPK increase to 120.7%, CK-MB at 135,7%, γ-GT to 54.72%, 98.7% for LDH, AST at 172.61% α-amylase to 119.58%, 82.99% for alkaline phosphatase. This indicates that the pathological process involved the cardiovascular, digestive, urinary, respiratory system, structural-metabolic disorders which occur against the background of hypoxic conditions and abuse of power, carbohydrate and protein metabolism. It should be noted that the activity of organ liver enzymes-ALT patients in the comparison group did not significantly increased compared with the control group, while the rate in patients of the group increased by 270.74%. Analysis of performance indicators show that in COPD in combination with hypertension observed a significant increase in AST, CK, CK-MB and γ-GT in comparison with the second group of patients (with isolated COPD) who virtually unchanged organ activity of liver enzymes-ALT. This data can be prognostically significant in substantiating the risk of complications in patients with comorbid pathology. It may indicate the extrapulmonary manifestations of COPD and leads to a significant voltage protective and adaptive mechanisms already isolated background COPD and reach their maximum values in patients with comorbidity pathology.
