RESEARCH IN THE ENDODONTIC AND THERAPEUTIC FIELDS OF APPLICATION OF INNOVATIVE BIOACTIVE CEMENT

Over the past decades there have been many changes in the way of performing endodontic treatment. The standard protocol has undergone several modifications, more so because of increased demand from the patients for saving their teeth and advances in material science and innovative equipments. It is an actual issue searching of optimal material for supporting of the process of the remineralization of dentine which stimulates the formation of solid tissues, provides the restoration of general integrity of the pulp and fosters the long-term preservation of its vitality. The aim of the work is to perform the research in the field of endodontic and therapeutic application of innovative bioactive dental cement – biodentine, as well as development of practical recommendations that have the aim to increase the clinical effectiveness of treatment results. To diagnose and control the effectiveness of the treatment results electrodontodiagnostic and radiovisiographic methods were used. The material was widely used during the work in the zone of tooth crown. 181 patients at the age of 19…65 have been under supervision currently. In each clinical case, after 1…3 months after completed treatment, at clinical and diagnostic examination, it was found out that certain teeth are symptomless and there are clearly visible signs of active healing process. It has been proven experimentally that boidentin is an effective material that has the capacity to support and maintain the vitality of the pulp. The positive effect of biodentin material based on calcium silicate is in the releasing of hydroxyl ions during the stage of hardening of the material. This reaction provides a high alkaline environment, where the pH is about 12.5. Therefore it stimulates the formation of substitutional dentine. However it was found out that Biodentin should not be recommended for work in the zone of the tooth root and in a case of restoration of large part of affected tooth as well as for treatment of permanent pulpitis. There are some practical recommendations for increasing clinical effectiveness of results of treatment. Some practical methods of carrying out therapeutic measures in a case of complications after endodontic treatment are proposed.
