EFFICACY OF A PROLONGED PHOSPHATIDYLCHOLINE REGIMEN IN PATIENTS WITH ESOPHAGEAL AND GASTRIC VARICEAL BLEEDING

Abstract. Improving the functional state of the liver and reducing the rate of disease progression are key objectives in the treatment of patients with esophageal and gastric variceal bleeding. The aim of the study was to evaluate the effectiveness of a prolonged regimen of phosphatidylcholine administration in the complex treatment of liver cirrhosis complicated by esophageal and gastric variceal bleeding. The prospective study included 62 patients who were randomly divided into two groups. The experimental group (32 patients) consisted of patients who were prescribed a prolonged regimen of phosphatidylcholine to maintain liver function. The comparison group (30 patients) included patients who did not receive these drugs. To assess the dynamics of changes in the functional state of the liver, we analyzed the following indices: total bilirubin, alanine aminotransferase, aspartate aminotransferase, gamma-glutamyltranspeptidase, alkaline phosphatase, albumin, prothrombin index. The length of hospital stay was significantly shorter in the experimental group (8.4±2.6 days) than in the comparison group (10.3±2.3 days). Patients in the experimental group had a tendency to faster progressive decrease in the intensity of the studied indices, compared with patients in the comparison group. Improvement in the functional state of the liver was also observed after discharge of patients from the experimental group from the hospital. In the comparison group, there was a slight improvement in laboratory parameters of liver function during the hospital stay. After discharge from the hospital and cessation of intensive care, no changes in laboratory indices were observed. The use of a prolonged regimen of phosphatidylcholine in the complex treatment of patients with liver cirrhosis complicated by esophageal and gastric variceal bleeding can significantly improve the basic clinical and laboratory parameters of liver function and reduce hospital stay. In the future it is planned to study the functional state of the liver in the dynamics at 6 and 12 months.
