TREATMENT OF GENITAL PROLAPSE IN WOMEN WITH CHRONIC DISEASES OF THE RESPIRATORY SYSTEM

The aim of the study is to increase the effectiveness of treatment and prevention of recurrence of genital prolapse in women with chronic respiratory diseases. Material and methods. To achieve this goal we examined 72 women, including 42 women with genital prolapse and 30 women without signs of genital prolapse (control group). 20 patients with genital prolapse (main group) suffered from chronic diseases of the respiratory system (chronic obstructive pulmonary disease, bronchial asthma, chronic bronchitis); 22 women with genital prolapse without signs of respiratory diseases were included into the comparison group. We studied the anamnesis of the disease, the nature and severity of chronic respiratory diseases, the presence of other extragenital pathology, the degree of genital prolapse and methods of its treatment, namely surgery: transvaginal extirpation of the uterus without appendages, anterior colporrhaphystosis, colpoperineorrhaphy with levatoroplasty, sacrospinal colpopexy with the use of the “Polimesh” endoprosthesis. Research results and discussion. Women with asthma had a significantly higher number of concomitant extragenital pathology in contrast to the comparison group, where patients often had gynecological pathology. The advantage of surgical treatment, especially with the use of Polymesh endoprosthesis, which is 60% resorbable, is one-step correction of genital prolapse with the elimination of pelvic floor defects. Therefore, when choosing a method of surgical treatment, it is necessary to take into account the cause of genital prolapse and to choose a personalized method of surgery for each case individually, especially in women with pathology of the respiratory system. Conclusion. A personalized approach in the treatment of genital prolapse in women with chronic pathology of the respiratory system helps to increase the effectiveness of surgical procedures and reduce recurrence. The use of Polymesh endoprosthesis for sacrocolpopexy reliably prevents recurrence, increases efficiency and can be recommended in general gynecological practice.
