GENDER DIFFERENCES IN THE REMODELING OF THE CARDIOVASCULAR SYSTEM IN ACUTE MYOCARDIAL INFARCTION AND THEIR RELATIONSHIP WITH THE BALANCE OF SEX HORMONES

Abstract. Methods. 159 patients (119 men and 40 women) aged from 18 to 85 years were examined (the average age was 57.5±0.83 years) with acute myocardial infarction (AMI) in the intensive care unit. The method of echocardiography (EchoCG), ultrasound scanning of the common carotid arteries (CCA) and the right brachial artery (RBA) (reactive hyperemia test (RHT)) was used. In the blood plasma, the content of the sex hormones testosterone (TST), estradiol (ESD) and the lipid profile of the blood were determined. Results. In men with a decrease in the level of TST and a simultaneous increase in the concentration of ESD in plasma, there was a significant increase in the thickness of the interventricular septum (TIS), the thickness of the posterior wall (TPW), the end-diastolic volume (EDV), the end-systolic size (ESS) of the left ventricle (LV), as well as the intima-media thickness complex (IMT) of the common carotid arteries (CCA) and significantly lower than the value of the interadventitial diameter (IAD) CCA compared to patients with a normal balance of sex hormones, or in some men with a decrease in the level of TST in comparison with patients, in which the level of ESD was increased. In women with a decrease in the concentration of ESD in plasma, there were significantly increased TIS, LV TPW, aortic opening diameter, as well as average values of IMT CCA and minimum values of IAD CCA, both in comparison with women with normal hormonal balance, and with an increase in the level of TST. The most frequent significant violations of the vasomotor function of the endothelium were observed among men with a decrease in the level of TST and / or with a simultaneous increase in the level of ESD, in women – with a decrease in the level of ESD. A change in the balance of sex hormones towards a decrease in the level of TST in men and a decrease in the level of ED in women can be considered as an additional marker of an unfavorable course of acute myocardial infarction due to more pronounced remodeling of the heart, common carotid arteries and a significant violation of the vasomotor activity of the endothelium.
