RECOMMENDATIONS OF THE IMPLEMENTATION OF INJECTION METHODS OF ANESTHESIA FOR THE TREATMENT OF ACUTE EXACERBATION OF CHRONIC PERIODONTITIS

Periodontitis is a disease of the periodontal inflammatory character in which the inflammatory pro-cess involves not only the periodont, but also the surrounding tissue, where perifocal inflammation is observed in acute periodontitis or an area perifocal inflammation in chronic periodontitis. The main tasks of the treatment of acute and exacerbated chronic periodontitis are early elimination of the in-flammatory process in the periodontal tissues, prevention the spread of the inflammatory process and elimination of pain. Dental interventions in the treatment of periodontal disease are accompanied by pain of a greater or lesser intensity therefore anesthesia is one of the actual problems of dentistry. The purpose of this article is to review the applied questions of an injection anesthesia for treatment of an acute exacerbation of chronic periodontitis. Premedication significantly facilitates the work of the dentist and has a posi-tive influencing on the choice and management of anesthesia. For the optimal choice of local anesthetic preparation and method of anesthesia before the injection it is neces-sary to estimate the general state of the patient, namely accompanying diseases. It is necessary to restricts using of local anesthetics for women in the first trimester of pregnancy, because placenta is complete formation by 14 – 16 weeks of pregnancy. Artikaine has a minimal degree of penetration through the placental barrier, so it’s the drug of choice for local anesthesia in pregnant woman. In the selection of anesthetic a dentist should to give preference to one firm of anesthetics, because it helps to choose a better dose of the drug, to predict power anesthesia and potential complications. To prevent the occurrence of pain reaction on the injection a surface anesthesia is recommended prior to injection (lidocaine 10%, Xylonor spray, Lidoskor spray) and inserting the needle into the tissue at an altitude of a deep breath. Speed of injection should be 1 ml per minute, allowing the body to adapt to its introduction and to minimize possible complications. For the prevention of pain and as a postfilling anti-inflammatory drug in the treatment of acute periodontitis and exacerbation of chronic periodontitis it is recommended to take a non-steroidal anti-inflammatory drug «Nise», 1tab. (100 mg) twice a day, morning and evening for 1 or 2 days. Anesthesia in acute periodontitis excludes the use intrapulp and intraligamentary anesthesia. Perfect local anesthetic does not exist, but a dentist can choose the drug that most closely matches purposes and tasks facing them. In addition, to prevent many local complications associated with the technique of anesthesia a doctor should constantly improve their knowledge and skills.
