Анотація:

Рак шлунка (РШ) залишається одним з найпоширеніших онкологічних захворювань і займає четверте місце по частоті в структурі онкозахворюваності. Агресивний перебіг раку шлунка, рання дисемінація вимагають розробки додаткових системних методів лікування. Ад’ювантна хіміотерапія знижує ризик смерті на 18,0% і покращує загальну виживаність на 6,0%. Ефективність АХТ не залежить від глибини інвазії пухлини в стінку шлунка. АХТ ефективніше при наявності метастазів у регіонарних лімфатичних вузлах. Використання АХТ статистично значимо зменшує кількість рецидивів. Проведення неоад’ювантної (НеоАХТ) поліхіміотерапії хворим з місцево розповсюдженими формами раку шлунка покращує віддалені результати лікування в порівнянні з хірургічної групою. П’ятирічна загальна виживаність підвищилась на 13% і склала відповідно 36%. Використання ХТ при поширеному РШ приводить до скорочення ризику смерті в порівнянні з симптоматичною терапією (відношення ризиків (ВР) 0,39; 95%, збільшення медіани загального виживання з 4,3 міс. до 11 міс. Перспективне використання цитостатиків: таксанів, іринотекану, препарату платини III покоління оксаліплатину, пероральним фторпіримідинами (капецитабин, UFT, S-1). При застосуванні режимів FOLFOX, FOLFIRI об’єктивна відповідь склала 31,8%, час до прогресування 5,8 міс. і загальне виживання 9,0 міс. Доцетаксел і паклітаксел в монотерапії I, II лінії показали безпосередню ефективність 11-24%. В поліхіміотерапії паклітаксел вмістні схеми показали об’єктивну відповідь 22-51%, з медіаною виживаності 6-14 міс. При НER2позитивному раку шлунка ефект від хіміотерапії підвищується на 12% при додаванні трастузумаба, а медіана виживання збільшується до 13,8 міс.
