АСОЦІАЦІЯ ЗМІН ПСИХОЛОГІЧНОГО СТАТУСУ ЖІНОК З АРТЕРІАЛЬНОЮ ГІПЕРТЕНЗІЄЮ, ОЖИРІННЯМ І ДІАСТОЛІЧНОЮ ДИСФУНКЦІЄЮ ЛІВОГО ШЛУНОЧКА СЕРЦЯ ЗІ СТУПЕНЕМ МЕТАБОЛІЧНИХ РОЗЛАДІВ

Анотація:

Цукровий діабет 2 типу є одним з найпоширеніших хронічних неінфекційних захворювань. У ряді досліджень показано, що стать тісно пов’язана з появою симптомів тривоги і депресії, причому у жінок розлади психологічного статусу спостерігаються в середньому втричі частіше, проте механізми цих асоціацій залишаються до кінця не вивченими. Метою дослідження було вивчення психологічного статусу жінок з АГ, ожирінням і діастолічною дисфункцією лівого шлуночка серця з різним ступенем метаболічних розладів. Об’єкт і методи. Обстежено 125 пацієнток віком 40-60 років з АГ і ДДЛШ. Когорта пацієнтів зі 100 осіб була поділена на групи в залежності від ІМТ і наявності ЦД. Групи 1, 2 і 3 склали пацієнтки без ЦД зі ступенями ожиріння I, II і III відповідно. До групи 4 потрапили пацієнтки з різним ступенем ожиріння і супутнім ЦД. Ще 25 пацієнток з нормальними показниками ІМТ без ЦД склали групу контролю. Для оцінки психологічного статусу використовувались шкали депресії Бека і тривожності Спілбергера-Ханіна. Результати опитування інтерпретувалися відповідно до встановлених рекомендацій. Статистична обробка даних проводилася з використанням пакету програм Statistica for Windows версії 6.0. Результати. Встановлено збільшення рівнів реактивної та особистісної тривожності, а також кількості балів за ШДБ, в залежності від збільшення ІМТ. За результатами опитувальника Бека не було виявлено в групах порівняння жодного пацієнта, який набрав більше 19 балів (значна депресія). В групі з супутнім ЦД відзначалося збільшення в 1,2 рази кількості пацієнтів, які набрали від 10 до 18 балів. У жінок з ЦД спостерігалося збільшення кількості хворих з помірним ступенем особистісної тривожності в 1,2 рази в порівнянні з групою без ЦД. Пацієнток, які набрали від 31 до 45 балів (помірна ступінь) за ШРТ в групі з ЦД було в 1,5 рази більше, ніж в групі без супутнього ЦД, також визначалося збільшення числа пацієнток з високим ступенем реактивної тривожності в групі з супутнім ЦД в порівнянні з особами без ЦД в 1,3 рази. Висновки. Жінки з АГ, ожирінням і ДДЛШ серця характеризуються підвищенням рівнів реактивної та особистісної тривожності, а також збільшенням кількості балів за шкалою депресії Бека, ступінь яких збільшується по мірі наростання метаболічних розладів. У жінок з АГ, ОЖ і ДДЛШ наявність супутнього ЦД 2 типу асоціюється зі збільшенням частоти помірної і високої ступенів тривожності і підвищеною кількістю балів за шкалою депресії Бека в порівнянні з хворими, які не мають супутнього ЦД.
